Joining the Practice
Our practice list is open and we welcome new
patients. Any person regardless of race, gender, social
class, age, religion, sexual orientation or appearance,
disability or medical condition can apply to join the
Practice providing the live within the Practice Boundary,
are British citizens and the practice list is open.
The Map displayed
here (and in
the Reception area) depicts the area covered by the
Practice. If you reside within the area shown it is likely
that you will be accepted as a new patient.
If you are not sure whether you meet the criteria please ask
at reception.
If you fulfil these criteria and wish to join please fill in
the GMS1 form which can be downloaded by clicking on the
link below and hand it in at the reception.
Once the paperwork has been checked we will accept you onto
our list and apply to the Registration Department via PCT
electronic messaging system to transfer your notes to the
Practice.
If at any point your application is found not to fit our
criteria we will be unable to accept the application and
will inform you of the reason.
