All practices have a contractual requirement to allow patients to complete tasks on line. From 31st March 2016 the practice will increase the number of actions possible on line so that patients will have the ability to:
- Book / Cancel appointments
- Order repeat medication
- Have access to their Summary Care Record
· Have access to their Detailed Coded Medical Record
Should you wish to take advantage of any of these on-line services you will need to come into the Practice to sign up. You will need an up to date email address and two forms of documentation must be provided as evidence of identity, one of which must contain a photograph. Acceptable documents include passports, driving licences and bank statements. Recent household/utility bills dated within 3 months are also acceptable.
The practice accepts applications from patients as well as their proxy. Proxy access refers to access to online services by somebody acting on behalf of the patient with the patients consent. A competent patient can choose and give written consent for relatives and/or carers to have proxy access to their medical record. Separate application forms are available for this purpose. Please enquire at Reception if you require this service.
